What you receive
Two documents, emailed to you as PDFs and plain text.
- A carer evidence statement. Your account of care needs set out under each of the 12 care domains the assessment uses, built from your answers, and in your own words wherever you add them. It records frequency, duration, how many people are needed and what has happened when help was late or missing. It does not invent clinical language and it never states a level.
- One letter, matched to your stage. A Checklist reconsideration letter if the person was screened out at Checklist stage, or a review request letter if a full assessment has already produced a not eligible or disputed outcome. If no assessment has happened yet, you receive a preparing for assessment note instead.

Who it is for
- A family member, friend or representative of the person being assessed.
- You were screened out at Checklist stage, or a full assessment produced an outcome you dispute, or an assessment is coming and you want to be ready for it.
- You are prepared to spend 20 to 40 minutes describing the care in detail, in stages if you prefer.
What it is not
- Not representation. Nobody from us attends the assessment, speaks for you, or corresponds with the NHS or your local authority.
- Not legal, clinical or benefits advice. We draft documents from what you tell us.
- Not a prediction. Eligibility for NHS Continuing Healthcare is decided by the integrated care board, informed by a multidisciplinary team assessment. We are not part of that assessment and cannot attend it, and nothing here predicts or implies a decision.
- Not for Wales, Scotland or Northern Ireland. The pack is written to the England national framework, Checklist and Decision Support Tool. The other three nations run separate schemes with different paperwork, so it would misdirect an assessment there.
- Not for the Fast Track pathway. If the person's condition is deteriorating rapidly and may be entering a terminal phase, that is a different, faster route completed by a clinician. Ask the clinical team, and use Age UK, Carers UK or Citizens Advice. We will not take payment for that situation. What the reviewed tier adds
- A caseworker will handle your case, and our document engine will do the drafting.
- Every claim will be checked back to the answers you gave, and to your own words wherever you added them, and anything that is not traceable to them will be taken out.
- A short written review note, signed with your caseworker's name, will be included with your pack.
- If the pack does not meet the standard it will be held rather than sent, and you will hear from a person about it.
How this works

Review means a person checks the document against what you told us, removes anything that is not traceable to your answers, and checks the deadline and the route are stated correctly. It is not representation, it is not legal or benefits advice, and nobody can tell you what a decision maker will do.
The reviewer can approve your pack or hold it back. They do not rewrite it, so nothing can be added to your document that did not come from you.
The NHS Continuing Healthcare process is free and free help exists. Age UK, Carers UK and Citizens Advice all support families through it, and your local authority and integrated care board must give you information about it. This pack is optional.

What do I receive?

What if neither stage applies yet?
If no assessment has happened yet, you receive the evidence statement plus a preparing for assessment note instead of a letter, because there is nothing yet to reconsider or review.
Does it say whether we are eligible?
No, and it never will. Eligibility for NHS Continuing Healthcare is decided by the integrated care board, informed by a multidisciplinary team assessment. We are not part of that assessment and cannot attend it. The documents describe care needs, they do not score them and they do not predict a decision.
Why does the intake take 20 to 40 minutes?
Because it asks about all 12 care domains, one at a time, in the order the assessment uses. It saves as you go, you can stop and return using a link we email you, and you can dictate answers instead of typing them.
Can I do this as the person's representative?
Yes, and most people do. You give your own details as the representative, the person being assessed is named as the subject of the documents, and you record your relationship and the basis on which you are acting. Your name never appears as the source of a clinical claim.
What if the person needs the Fast Track pathway?
Then this pack is not for you and we will not sell it to you. The Fast Track pathway is for a rapidly deteriorating condition that may be entering a terminal phase, it is completed by an appropriate clinician and it is meant to be decided quickly. Ask the clinical team directly and use the free routes.
Is this legal or benefits advice?
No. We draft documents from what you tell us and give general information with sources. We are not a solicitor, an advocate or a benefits adviser, we do not contact the NHS or your local authority on your behalf, and we do not attend meetings.
